Cancellation Policy
Intake I hereby authorize Elmira Farnoosh to charge 75 $ for the virtual appointment. I provided. I understand that these funds will be drafted on the due date reflected on my statement, or if the due date falls on a week end or holiday, the funds will be drafted on the next business day. I hereby understand that this authorization will remain in effect until the amount is paid in full which is one time payment of $75 in total to confirm my online appointment. I agree to notify Elmira Farnoosh any changes in my financial situation that may lead to not being able to fulfill the payment prior to my appointment. I understand that if I fail to make these payments, I’ll cancel the appointment that I made online and I’ll reschedule for another day. I understand all payments made to Elmira Farnoosh for virtual appointment are non refundable under any circumstances. I understand this contract is legally binding.
